CANADIAN, Okla. – Several employees at a controversial drug and alcohol treatment facility in southeast Oklahoma have had their counseling certifications revoked.

The National Association of Forensic Counselors announced Arrowhead CEO Gary Smith and several employees at the Narconon Arrowhead treatment facility had their certifications revoked Saturday.

Those at the facility near Lake Eufaula will not be eligible to be re-certified.

The non-profit treatment facility has been the subject of an ongoing investigation after three patients were found dead in less than one year.

Authorities have not released if the revocations are related to the investigation.
